The autism care navigation service is for adults who live in Brent and who have either a diagnosis of autism or are on the waiting list for an autism assessment.
People can use this service to get information about local health and social care services, and to find particular services that will meet their needs. This could include statutory services or voluntary/community groups. The autism care navigator can also help people access services, for instance by making a referral.
Information and feedback shared with the autism care navigator is used to help improve the experience of people using local services. For instance, feedback about the autism diagnosis process is collected and shared regularly with the service provider to help drive improvements.
The autism care navigator works closely with our Healthwatch Brent and My Health, My Choice services. This means that people who aren’t eligible for help from the care navigation service can still get support to understand their rights and access health or social care.
